Iron Deficiency in Athletes: More Than Just Fatigue

Iron plays a central role in oxygen transport, energy production and muscle function. For athletes, even mild iron deficiency can reduce endurance, slow recovery and impair strength — often before anaemia develops.

Why Athletes Are at Higher Risk

Endurance athletes, particularly runners, are more prone to iron deficiency due to:

  • Increased iron losses (including gastrointestinal loss and sweat)

  • Repetitive impact causing red blood cell breakdown

  • Exercise-induced inflammation reducing iron absorption

  • Higher overall physiological demand

Female athletes and adolescents are at particular risk, especially during periods of heavy training or growth.

Symptoms May be Subtle

Iron deficiency in athletes does not always present dramatically. Common signs include:

  • Unexplained fatigue

  • Reduced stamina

  • Slower recovery between sessions

  • Declining performance despite consistent training

  • Increased perceived effort at usual workloads

Importantly, haemoglobin can remain in the normal range while iron stores are low enough to affect performance.

Proper Assessment Matters

Checking iron status involves more than a standard blood count. Ferritin, transferrin saturation and inflammatory markers all help interpret results accurately — particularly in athletes where inflammation can affect readings.

Identifying the cause of deficiency is also important, rather than simply replacing iron.
